I hope I did not miss a feature already implemented ... if a file cannot be renamed because there is already a file with the new filename, it is not processed (the file is not renamed an an entry is generated in the log).
I suggest a variable (for example %duplicate%) which is empty when the new filename does not exist and is incremented by 1 when the new filename already exists.
This would be a great help if you rename pictures using the EXIF date/time function and you have pictures taken at the same time. Using this feature you would generate "Picture_2008-08-08_11-00-00.jpg" for the first picture taken at 11 o'clock and the second one would get "Picture_2008-08-08_11-00-00_1.jpg" and so on. Without this feature, you would have to find out the date/time value and rename the files manually.

This is an interesting idea.
Actually some operations like "use date & time" (but not the Exif one) have an option to append a suffix, but it would be better with a tag (as you suggested) allowing to place it anywhere, with eventually some extra text, e.g. (1) instead of just 1.
So I will add that on my to-do list. But unfortunately I cannot promise that it will be added soon, due to the already long list of interesting features to add
