Trying to create a template

I keep getting a calculate error when i preview the template
I want to list all records in 4 columns
1 = origional title
2 = media label
3 = borrower
4 = media type.
can anyone help create this template?

Can you send/post the template? Or at least a screen capture of it when shown it the Designer? Maybe it could help to find what is the error, as creating such template is not much different from the style of those already provided with the program.
