For now you can't group by the first letter.

I will try to add more options for grouping in futur version:
- take original selected field value to make groups (by default)
- take only first letter of value to make groups.
- take only two first letters of value to make groups
- take only three first letters of value to make groups
- take integer/real value to unit to make groups
- take integer/real value to ten to make groups
- take integer/real value to hundred to make groups

I do not know if I understand correctly but the problem isn't to add the first or second letter, but allow you to create groups within groups, as well as the subfolder in the Windows folders

But with my proposal above you will have groups with just first letter of media label value.
If you want to see media label instead of translated title you have two solutions:
- you can replace translated title by media label value in options
- you can use grid mode instead normal list and put media label column at left

Having said that I would say that for me is not a good solution and if you could ask to insert the function that would open up endless ways to customize the program. I think a lot of work hours can lead to big savings in the future work.
